By participating in REST services (i.e. events/website/consults/courses/ emails/etc.) I acknowledge that REST staff are not licensed healthcare professionals and our services do not replace the care of licensed healthcare professionals. Our ministry is in no way to be construed or substituted as psychological counseling or any other type of therapy or medical advice. REST staff will at all times exercise our best professional efforts, skills and care. Release, Waiver and Assumption of Risk *

This is a legally binding release, waiver, and assumption of risk made by me (the participant) to REST Ministry Network LLC (the company). By signing this waiver agreement, I am agreeing to the following terms: 1) The participant agrees that their participation in this event is voluntary. Additionally, the participant agrees to assume any risk that is associated with participating in the event and releases the company from any and all claims of damage and loss that may occur as a result of participation. The release is without limitation and includes the personal injury fees, loss of personal property, attorney's fees and any other losses that might occur to the participant. 2) The participant agrees to hold the company harmless against all claims of damage and loss including damage to the company's property caused by the participant. 3) The participant hereby warrants that they are physically fit and able to participate in this event without any undue risk. 4) the company maintains the right to refuse the participation of the participant in this event if they foresee that participation would risk damage to any party. 5) This waiver agreement is governed by the laws of the state of Nebraska. Any legal claims or lawsuits related to the participants in this event shall take place in courts located in the state of Nebraska. 6) The participant agrees that this waiver agreement shall remain in full force and take effect without change and that participation in this event is pursuit to the terms of the agreement.
